About Whole Health Primary Care
At the Whole Health Primary Care Clinic, quality healthcare involves taking care of both your mind and your body. We take care of your physical health care, and work closely with our colleagues at the Guidance Center for behavioral health needs, all on the same campus.
Our approach recognizes that mental health and physical health are deeply connected. By offering both under one roof, we make it easier for our clients to get the complete care they need.
Our clinic serves as a medical home where patients can receive routine check-ups, manage chronic conditions, and access preventive care, all while having seamless access to our full range of behavioral health services. We accept patients of all ages and most major insurance carriers including TRICARE.
We also have our own on-site lab, so clients can have their blood drawn right here — no separate trip to an outside lab.
Our providers include K. Ashton Moore, APRN, who specializes in primary care in rural health and is passionate about wellness and getting to the root cause of disease, and Ashley Crow, APRN, a wound care specialist who is passionate about DHS custody care and addiction services.
We are now offering Vitamin IV Therapy. Contact us at 479-785-9400 to learn more or schedule an appointment. Our clinic is open Monday through Friday, 8:00 AM to 4:30 PM, by appointment. We are located at 3117 So. 70th Street, Fort Smith, AR 72903.
